Your Office Visit

Please arrive 15 minutes earlier than your scheduled appointment.

Items to bring with you:
1. Picture Identification
2. Insurance Card
3. Past medical records
4. List of all medications used or bring in your medication bottles
5. Any imaging studies (X-rays, CT scans or MRI films) involving the medical problems for your visit.
6. Insurance card
7. Completed information forms.  These can also be completed at the office.

Please call to cancel an appointment with at least 24 hours notice. No-shows may be charged a fee.
